This is the form that needs to accompany your module on its journey for repair. Be it warranty or otherwise. But before you fill this out and send off your precious posession make sure to call in to Tech Support to be sure that it needs to be sent in.

The form is also fillable so you can type your information in and then simply print out the copy to sign. Once completed, please email it in to us or you can also fill it out and leave it with the vehicle when dropping it off after hours.
